The contact form was downloaded from https://app.web3forms.com/dashboard and looks like attached template. But when I put the code in an htm file it looks like contact_h_251217.jpg. There is some CSS or other formatting needed to make the labels, textboxes and submission laid out more like Template_251217.pg Doesn't have to be perfect but just a little more like the template.
