My NDIS-focused healthcare site is overdue for a refresh. I want the overall look modernised, but the biggest priority is the referral page. Right now it feels clunky and the copy is out-of-date. I need a clean, accessible interface that speaks to allied-health and mental-health referrers and meets standard WCAG colour-contrast guidelines. On that page I also need: • a streamlined form for incoming referrals • built-in referral tracking so I can see status and source at a glance • automated email or SMS notifications to both the referrer and our team when a form is submitted All data from the form must flow reliably into my existing Zapier automation. The Zap is already half-built but keeps erroring when it tries to write to the website database—so troubleshooting and finishing that connection is part of the job. If you spot a simpler way to wire the hand-off (Webhooks, REST hooks, Make.com, etc.) I’m open to suggestions as long as it stays low-code and easy for me to maintain. Deliverables will be: 1. Redesigned referral page (UI + accurate content). 2. Working form with tracking and notifications. 3. Repaired or rebuilt Zapier workflow, tested end-to-end.
