I need my existing booking form (see https://londonheartsandhomes.ca/booking-form/) rebuilt as a streamlined, mobile-friendly, three-step WordPress form. All copy, field names, and the flat-rate tables are already organized inside three PDFs that I will supply as soon as we start. Step 1 must display 6 categories attach the correct flat-rate price to each category and depending on the number of rooms that customer choose, the price as soon as it is chosen, showing the running total before the user advances. Step 2 asking for 12 extra services if they need to select. Step 3 contact information and allows comments before submission and then sends a branded confirmation email to both the user and me. Please set the form up so I can adjust rates later from the WordPress dashboard. A clean back-end interface (Gravity Forms, WPForms, or something equally intuitive) and responsive front-end styling are essential.
